Paul -- I'm finding that when I enter new references now, the author names are getting indexed but the publication years aren't. So for example if I do a quick search on "Jones Smith 2003" I'll perhaps get one or two hits, but if I do a search on "Jones Smith" I'll get a lot more, including some published in 2003. I've tried rebuilding the search index and the whole database, and nothing helps.

Hugh, please turn on "Auto update File As field" on the "Tools | Options" window "Database" tab. Then when new records are added, File As field will be added automatically. Thanks.
