Importing citations

I am having no luck in importing citations directly from various medical journal websites. The manual gives me very limited information about how to download - it seems as though this should be straightforward. Usually when I try to save a citation from a journal website, it does not get any sort of extension (like .enl) and if I try and import it into Biblioexpress then I am told it is an invalid file format. I am trying to download using EndNote format. Some journals talk of a RIS format that should work with EndNote, Reference Manager, Procite. Where do I find an idiot's guide to importing citations from journal websites and/or what file extensions should the citations be saved as?

Biblioscape supports Direct

Biblioscape supports Direct Export. It can import references easily in this case. For BiblioExpress, it can only import references in EndNote Refer format. It cannot import *.enl or *.ris files.
