Document (ref_doc) field not visible for References

According the the manual, the Document (ref_doc) field can be used for entering the full text of a reference. When I double-click a reference, and click the 'All fields' button to display all available fields, I don't see any field called 'document' (or ref-doc).

Is there something I need to do in order to make this field visible for me to use?

The "Ref_doc" field can be accessed on the "Document" tab of the reference editor window.

I can't find a 'document

I can't find a 'document tab' in the reference editor window. Can you explain further where this is located?
